A collaborative team approach helps to dramatically improve care transitions and reduce re-hospitalization rates. We offer a wide range of high quality health care services with a “Rehab to Home” focus.

Rehabilitation and Specialized Programs

As leaders in post-hospital surgical care and rehabilitation, our highly trained therapy team offers individualized treatment plans utilizing the most innovative technologies.

Care Planning

The care plan begins with a comprehensive assessment of the patient, including level of function and social, mental and medical conditions. The plan focuses on maximizing independence and functioning to the highest level possible. The care plan is revised every quarter or when the resident’s condition changes. Families are strongly encouraged to participate in quarterly care plan sessions.

OmniVR™

Virtual Rehabilitation System

HCF OmniVR now offers the first virtual rehabilitation system developed to accommodate medically complex patients. An advanced 3D camera tracks the patients precise “real-time” movements and allow them to interact in a virtual world. The system includes a variety of “skilled” exercise programs for physical, occupational and speech therapy applications. It can also accommodate multiple patients-allowing for group therapy.

Group therapy is not only excellent for enhancing social interactions and exercise activity, it’s also enjoyable. Patients enjoy performing virtual exercises and activities, are ultimately more motivated. Therefore, the results of virtual rehabilitation treatment can be equal or even exceed results achieved with traditional exercise therapy alone.

OmniCycle™

The OmniCycle can assist in treating patients with a wide range of conditions, from short term rehab to patients with chronic conditions. The OmniCycle rehabilitation system has a smart motor which helps patients who have limited strength, endurance, or muscle control. Learn more details about our OmniCycle in this short video.

What is Aquatic Therapy?

Aquatic therapy is a skilled rehabilitation method, supervised by a licensed therapist that focuses on therapeutic exercises performed in a warm pool environment. Aquatic therapy is appropriate for patients of almost any age and is beneficial for a wide variety of conditions and injuries. Aquatic therapy is especially helpful but not limited to patients who have difficulty with weight bearing activities. The water’s natural state of buoyancy provides support with bearing weight while decreasing the stress placed on the joints.

What are the benefits of Aquatic Therapy?

  • Reduced pain with movement due to the decreased stress placed on joints
  • Decreased pain when relaxing muscles in the warmth of the water
  • Increased strength and endurance through the light resistance of the water
  • Improved circulation
  • Minimized swelling
  • Improved flexibility
  • Improved body mechanics and postural stabilization
  • Increased aerobic capacity and endurance training

Who may benefit from Aquatic Therapy?

Individuals with:

  • Arthritic pain
  • Chronic pain
  • Joint replacements
  • Neurological conditions
  • Orthopedic injuries/conditions
  • Poor balance/coordination
  • Poor flexibility and stiffness
  • Generalized weakness
